About the Role
RisingAssist is seeking a knowledgeable and detail-oriented Qualified Developmental Disabilities Professional (QDDP) to oversee the overall coordination and implementation of services for individuals with developmental disabilities. This role includes developing, monitoring, and updating each individual’s Person-Centered Plan (ISP), ensuring services align with identified goals, and maintaining compliance with all applicable DBHDS and Medicaid requirements. The QDDP serves as a key liaison between the individual, family members, support staff, and other service providers to ensure continuity of care and effective communication. In addition, the QDDP conducts ongoing assessments, reviews progress toward goals and ensures that services are delivered in a way that is consistent, individualized, and outcome-focused, promoting the highest possible level of independence and quality of life for each person served.
Requirements
Minimum Qualifications
Bachelor's degree in Human Services or related field
Knowledge of person-centered planning
Strong documentation and organizational skills
Perferred Skills
Experience working with ID/DD population
