Our Program
We offer a structured, person-centered program that focus on life skills, social engagement, and personal development.”

Life Skills Training: Daily living skills including personal care, organization, and routines.

Recreational Activities: Arts, games, group activities, and community outings to encourage social connection.

Behavioral Support: Individualized support plans tailored to each participant’s needs.

Health & Wellness: Activities promoting physical, mental, and emotional well-being.


Eligibility
To qualify for services at RisingAssist, individuals must:

Be 18 years of age or older

Be approved for a Virginia Medicaid Waiver, including:

Community Living (CL) Waiver
-
Family & Individual Supports (FIS) Waiver
-
Building Independence (BI) Waiver
-
CCC Plus Waiver (when Adult Day Support is authorized)

Have Adult Day Support services authorized in their Individual Support Plan (ISP)

Needs structured daytime support to enhance independence, social engagement, and daily living skills

Be able to participate safely in a group-based, supervised setting
Eligibility alone does not define acceptance. We focus on person-centered care, ensuring each participant’s strengths, preferences, and goals are respected. Program participation is based on the individual’s ability to benefit from Adult Day Support services as outlined in their ISP.
